Before making an appointment, please read our important patient reminders.

- If this is your first appointment, and you are having Skin Testing please remember you are not to take any Antihistamines for 4 days prior to your appointment. ( If you don’t know what an Antihistamine is, please call our office to discuss 630-584-6127)
- Aetna insurance does not allow Skin Testing and Consultations on the same day. You will need two separate appointments per your insurance policy.
- All appointments that are for Biologics such as Tezspire, Nucala, Dupixent, Xolair or Fasenra, please call the office an hour before your arrival to have your biologic prepared
- Please bring the following items/documents with you to your appointment
- Valid Photo ID
A valid photo ID is required to verify your identity. You cannot be seen for your appointment and will be asked to reschedule if you do not have a photo ID with you.
- Current Insurance Card
A valid insurance card is required in order for us to submit claims to your insurance. If you do not have an insurance card, then you will be self-pay and required to pay in total at the time of service.
- List of Current Medications
All Co-pays and Balances are due at the time of service. * You can be turned away for non-payment.
At Asthma and Allergy Wellness Center, you are required to wear a mask at all times while inside the office.
